Puppy Deposit Form
Puppies must be chosen prior to week 4 or the date determined by breeder.
Puppies must be selected by picture or video only, no onsite visitations prior to pick up or until puppies have been given their first vaccination.
A charge of $25 per day will be applied for boarding beyond pick up date determined by breeder.
Puppies must be picked up on the pickup date and any transpiration will be an additional fee.
Deposits are $500 - Total cost of puppy or Remaining amount to be paid after deposit if received.
Deposits are refundable until a puppy is chosen.
